Waiver Agreement

Member and studio acknowledge that the services, instruction, and use of equipment as provided for under this agreement entails some unavoidable risk of personal injury and harm, which is indigenous to physical contact sports, particularly those involving the martial arts. It is further expressly agreed that all instruction, services, consultation, and use of equipment and facilities as it pertains to martial arts training shall be undertaken by Member at Members' sole risk, and that Member assumes the full responsibility for any such injuries or damage arising out of or in any way connected with his/her use of any of the services or facilities. Studio shall not be held liable for any claim, demand, injury, illness, expense, damage, action, or cause of action arising out of or connected with the use of any of the services or facilities of Studio(Wisconsin Martial Arts and Fitness Center North LLC, and Wisconsin Martial Arts and Fitness Center West,LLC), including those arising from acts of passive or active negligence on the part of the Studio, its servants, founders, instructors, agents, or employees of Member. Member, for himself and on behalf of himself, executors, administrators, and assigns, does hereby expressly forever release and discharge Studio, its successors of assigns, as well as officers, directors, founders, instructors, agents, employees, and members from all such claims, demands, injuries,illness,expenses, damage, action, or cause of action. Further, Studio is not responsible for any loss or injury to personal property of Member while on, about or as a result of being at the Studio. Member attests that as a result of Member's training at the Wisconsin Martial Art and Fitness Center, Member will not teach, instruct, or give any form of lesson or advice in any martial art under any name without first obtaining all written authorizations from the Wisconsin Martial Art &Fitness Center to do so. Member agrees to adhere to any additional rules regulations as posted form time to time at Studio.
